Stem cells entering heart can be tracked with nano-hitchhikers

Mar. 20, 2013 ? The promise of repairing damaged hearts through regenerative medicine -- infusing stem cells into the heart in the hope that these cells will replace worn out or damaged tissue -- has yet to meet with clinical success. But a highly sensitive visualization technique developed by Stanford University School of Medicine scientists may help speed that promise's realization.

The technique is described in a study published March 20 in Science Translational Medicine. Testing the new imaging method in humans is probably three to five years off.

Human and animal trials in which stem cells were injected into cardiac tissue to treat severe heart attacks or substantial heart failure have largely yielded poor results, said Sam Gambhir, PhD, MD, senior author of the study and professor and chair of radiology. "We're arguing that the failure is at least partly due to faulty initial placement," he said. "You can use ultrasound to visualize the needle through which you deliver stem cells to the heart. But once those cells leave the needle, you've lost track of them."

As a result, key questions go unanswered: Did the cells actually get to the heart wall? If they did, did they stay there, or did they diffuse away from the heart? If they got there and remained there, for how long did they stay alive? Did they replicate and develop into heart tissue?

"All stem cell researchers want to get the cells to the target site, but up until now they've had to shoot blindly," said Gambhir, who is also the Virginia and D.K. Ludwig Professor in Cancer Research and director of the Molecular Imaging Program at Stanford. "With this new technology, they wouldn't have to. For the first time, they would be able to observe in real time exactly where the stem cells they've injected are going and monitor them afterward. If you inject stem cells into a person and don't see improvement, this technique could help you figure out why and tweak your approach to make the therapy better."

Therapeutic stem cells' vague initial positioning is just part of the problem. No "signature" distinguishes these cells from other cells in the patient's body, so once released from the needle tip they can't be tracked afterward. If, in the weeks following stem cells' infusion into the heart, its beating rhythm or pumping prowess has failed to improve -- so far, more often the case than not -- you don't know why. That ambiguity, perpetuated by the absence of decent imaging tools, stifles researchers' ability to optimize their therapeutic approach.

The new technique employs a trick that marks stem cells so they can be tracked by standard ultrasound as they're squeezed out of the needle, allowing their more precise guidance to the spot they're intended to go, and then monitored by magnetic-resonance imaging for weeks afterward.

To make this possible, the Gambhir lab designed and produced a specialized imaging agent in the form of nanoparticles whose diameters clustered in the vicinity of just below one-third of a micron -- less than one-three-thousandth the width of a human hair, or one-thirtieth the diameter of a red blood cell. The acoustical characteristics of the nanoparticles' chief constituent, silica, allowed them to be visualized by ultrasound; they were also doped with the rare-earth element gadolinium, an MRI contrast agent.

The Stanford group showed that mesenchymal stem cells -- a class of cells often used in heart-regeneration research -- were able to ingest and store the nanoparticles without losing any of their ability to survive, replicate and differentiate into living heart cells. The nanoparticles were impregnated with a fluorescent material, so Gambhir's team could determine which mesenchymal stem cells gobbled them up. (Mesenchymal stem cells, which are able to differentiate into beating heart cells, can sometimes be harvested from the very patients about to undergo a procedure. This could, in principle, alleviate concerns about the cells being rejected by a patient's immune system.)

Upon infusing the imaging-agent-loaded stem cells from mice, pigs or humans into the hearts of healthy mice, the scientists could watch the cells via ultrasound after they left the needle tip and, therefore, better direct them to the targeted area of the heart wall. Two weeks later, the team could still get a strong MRI signal from the cells. (Eventually, the continued division of the healthy infused stem cells diluted the signal to below the MRI detection limit.)

"There was some early skepticism about whether this would work," said the study's lead author, Jesse Jokerst, PhD, a postdoctoral scholar in Gambhir's lab. "Because the particles were so small, we weren't expecting much signal strength." But once ingested, the particles clumped together inside the cells, reflecting ultrasound waves much more dramatically and providing a surprisingly strong signal, Jokerst said.

"We got lucky," said Gambhir, who is also a member of the Stanford Cancer Institute. "Sometimes taking risks pays off."

Using the new agent, the Stanford investigators were able to detect as few as 70,000 of the stem cells by ultrasound and as few as 250,000 by MRI. Either number is tiny in comparison to the tens or hundreds of millions of stem cells being infused into human hearts in clinical trials today, Gambhir said.

No signs of toxicity or behavior differences were seen in mice receiving the agent-containing stem cells compared with control animals receiving stem cells without the agent, he said. The U.S. Food and Drug Administration has approved nanoparticles for other applications, and silicon is considered relatively nontoxic and has been used in the clinic. Gadolinium, which can be toxic at high doses, is clinically approved by the FDA in doses much greater than would be necessary for this new imaging procedure, even if hundreds of millions of stem cells were involved. Still, further toxicity tests will be needed, said Gambhir.

The mice used in this study were all healthy, Jokerst pointed out. The next step, he said, is tests on mice or pigs with analogs of human heart damage, as well as detailed toxicity studies of the nanoparticles.

Stem-cell therapy for damaged hearts won't be cheap anytime in the foreseeable future; it will certainly cost several tens of thousands of dollars per procedure. But, said Gambhir, a successful intervention could be cost-effective because, as a one-time delivery, it could eliminate or at least diminish a lifetime of constant drug administration. The new imaging technique might add another $2,500 to such a procedure, but could also greatly improve the odds of its success, he said. The approach could also be applied to regenerative procedures in other organs, such as the liver.

Stanford has filed for provisional patents on intellectual property associated with this research.

Don't Be Scared, It's Just A Bag Of Dried Beans: How To Cook Them

In our last installment of "Don't Be Scared, It's Just ____," we gave you our favorite one (or two) ingredient recipe: whipped cream. If you can handle whipping cream to perfectly soft peaks, you can absolutely handle our topic for today's edition.

Dried beans and I are having a bit of a kitchen love affair right now. But it hasn't always been this way. For most of my cooking life up to this point, I've been firmly, wholeheartedly terrified of them. Three things helped turn this culture of avoidance into an all-out season of appreciation: 1) my husband's general willingness to, through trial and error, actually make a few pots of really excellent dried beans, 2) the discovery of Rancho Gordo beans (which cook faster and taste better than any other bean I've ever tangoed with) and 3) Tamar Adler. More specifically, an excerpt from Tamar Adler's An Everlasting Meal, tucked away inside Best Food Writing 2012, in which she waxes particularly poetic about how nourishing and satisfying a pot of beans can be.

The biggest lie that every disciple of dried beans has ever told you is that you have to soak them overnight. You can soak them overnight. You should, if you have the foresight. Let me confess something to you: I have never had the foresight to soak dried beans overnight. The quick-soak method is your friend. As Adler puts it: "If you didn't put two cups of beans in a pot of cold water last night, get on the bandwagon today by putting them in a pot, covering them with five inches of water, bringing it to a boil, turning off the heat, and leaving them sitting in hot water, covered, for an hour. Then drain them and cover them with new water." (The draining is essential, as that water contains all the parts of beans beans that make them the musical fruit.)

Okay, so you've covered your beans in new water -- now what? This part is a bit like making stock. Your beans are going to taste like whatever you flavor this next batch of water with. There should be a bit of salt (you can always adjust later, so use a light hand), pepper, any herbs you have on hand, a chunk of carrot, celery, fennel, onion, a Parmesan rind.

Most importantly, as Adler puts it, there needs to be an "immoderate, Tuscan amount of olive oil," which I've taken to mean as much as I think is enough, and then a bit more. The major argument for using dried beans rather than canned is that you get to dictate what these beans taste like. Canned beans are great in a pinch, and I still use them when time is a concern or they are not the star of the dish, but they all just taste like canned beans. Dried beans, cooked in this way taste like vegetables. Like they grew out of dirt. And then drank a bunch of olive oil and herbs and swelled up like balloons full of flavor. The older your beans are, the longer they'll take, so if you pull a giant bag off the supermarket shelf, you'll be able to make them delicious, but it will just take a little more time.

How long should you cook these? One more bit of advice from Adler: when five beans eaten straight from the pot are tender and full of flavor. If you're using Rancho Gordo beans, it will take less time than you think. Plan for an hour or two.

These are not the kinds of beans you'll want to hide in any old soup or salad. These are the kinds of beans you'll want to eat out of a bowl, topped maybe with some avocado and feta, with another immoderately Tuscan glug of olive oil over the top. You'll eat these for days. And you'll feel pretty proud that you made what started out as colorful little rocks taste so delicious and feel so healthy. Not to mention you've saved yourself a boatload of money on beans. Deep breaths, you did it.

Fencing

Hedges are not the best idea because they take a long time to grow and are not as reliable. Wooden fences are a better option. 'Teen Mom' star Farrah Abraham arrested for DUI

Another misstep for another "Teen Mom" star. Farrah Abraham was arrested for a DUI early Tuesday morning in Omaha, Neb., police confirm to E! News.

The 20-year-old reality star was pulled over after almost hitting a police cruiser. She was then administered a field sobriety test and blew a 0.147. (The legal limit is 0.08.) Her 4-year-old daughter, Sophia, was reportedly not in the car.

On Wednesday morning, Farrah took to Twitter in a halfhearted attempt to set the record straight.

"It's Amazing what people believe & makeup (LOVE MY LIFE)" she wrote. "#Thank #God #I'mSuccessful & I don't care about drama! #HaveAGreatDay."

LG suspects Samsung of infringing its eye-tracking patents with the Galaxy S 4

Samsung's Galaxy S 4 isn't even available yet, but already it's being eyed for possible patent infringement. According to a report from Korea's Yonhap News, LG suspects the S 4 might violate eye-tracking patents used in the Optimus G Pro. At the crux of this squabble is Samsung's Smart Pause feature, which LG finds similar to its Smart Video technology. Chiefly, LG is focusing on a patent it applied for in 2009, though the company also plans to investigate whether Samsung infringed other eye-tracking patents dating back to 2005. So far, of course, Samsung has denied any wrongdoing, saying its eye-tracking tech is implemented differently and is based on proprietary technology. Given that the phone isn't even out yet, we'll leave it to LG to do its due diligence before accusing Samsung in court.

Description of the Position

The Vice Provost for Global Affairs provides strategic and innovative thinking regarding internationalization and global partnerships. The scope of the position is institutional and includes promoting globalization inside and outside Ohio University in research and scholarship; chairing the Senior International Management Team; serving as a resource for curriculum innovations and integration; encouraging student and faculty diversity; advancing collaborative international initiatives including enrollment efforts and the institution?s international strategic plan; advising university leadership on matters relating to international protocol; and actively engaging in fundraising. The Vice Provost works in partnership with the deans, other academic planning unit heads, and senior staff in the Office of the Executive Vice President and Provost. S/he is responsible for helping them to achieve their strategic goals.

The Vice Provost oversees the Center for International Studies, an academic unit charged with internationalizing the Ohio University learning and living community and infusing global education into the curricula on campuses and abroad. High quality, globally focused undergraduate and graduate degrees and programs are at the core of our mission, serving as a catalyst for interdisciplinary and experiential engagement among faculty and students across the institution. Reporting to the Executive Vice President and Provost, the Vice Provost for Global Affairs will possess the following qualifications:
